This form can be used to request funds from PTO as well as reimbursement for previously approved purchases. Please fill out as completely as you can and attach available information. We’ll reach out to follow up!

Submission Instructions
If you’re requesting PTO approval to purchase (or have purchased) an item, submit as Request for Approval – Not Yet Purchased. We’ll begin the research and approval process right away.
If you’ve already purchased an item and are requesting PTO consider reimbursing for the expense, submit as Request for Reimbursement – Not Yet Approved. We’ll begin the research and approval process right away.
If you’re submitting for reimbursement for a previously approved item, submit as Request for Reimbursement – Previously Approved.
